The boy is standing farther in the background. If you look closely you can barely make out the edge of whatever object it is he is leaning on. The camera man is close to the normal sized boar, and the perspective fools the eye.
All photographs of this supposed boar are done in the same way. This is why you will not see a single shot of the boar in actual physical contact with a person.

what he is leaning on is his left knee. he may be further back than the boar but that doesnt change the fact that that is a huge boar. The head is biggest i have ever seen, i have seen hundreds of hogs in the 4 to 5 hundred pound range and none look this big no matter where the hunter stands. not an fake, unless you live in new york and have never shot a hog much less seen a real one.
